you live in Calumet, Outagamie, or Winnebago counties
you can become a Health Center patient. At your first
visit please bring proof of family income, address,
photo ID on to the Health Center.
All medical insurances, including Medicare and Medicaid,
Badgercare (Forward card) are accepted.
No Insurance? Discount fees are available with based
on income and family size.
